How does elder care navigation work?

Our elder care navigation service at Your Legacy Legal Care® coordinates and assists with the move to a new elder care facility after we have prepared a tailored elder care plan. During this process, we may:
- Coordinate a client’s move to an elder care facility
- Craft or adjust care plans for elderly clients who choose to remain at home
- Modify plans as needed, as preferences may change after the transition to the elder care facility
Because some clients choose to remain in their homes, we also often help to arrange in-home services to fulfill their care plans and meet their needs.
